Human in the loop

Designed checkpoints where a person approves or corrects before the system acts.

Human oversight converts an autonomous system into an accountable one. The design question is which actions require approval, not whether a human is nominally involved.

Placement matters: approval before a consequential action is a control; review after the fact is a report.

The design detail that decides whether it works is how much context the human gets. An approval screen showing only the proposed action produces rubber-stamping within a week. One showing the reasoning, the source evidence and what the system was uncertain about produces actual review, and the difference is entirely in the interface.

Commonly misunderstood: A rubber-stamp approval step is worse than none. It manufactures the appearance of control without the substance, which is precisely what an auditor looks for.

Where this shows up in our work

Human in the loop is not an abstraction for us. It is a decision we make on live projects. It shows up most directly in ai governance & compliance, agentic workflow automation, where getting it wrong has a cost someone can measure.

If you are evaluating a vendor on this, the useful question is not whether they can define the term. It is what they measure, what they would refuse to do, and what happens in their system when the assumption behind human in the loop stops holding.
